Alcohol and Other Drugs Continuing Coordinated Care - Western NSW

Service Description

The AOD Continuing Coordinated Care (CCC) Service in Western NSW provides care coordination and wraparound services for people with a substance use disorder who are receiving community-based treatment, while also experiencing a high level of need. CCC supports clients with intensive support to maintain engagement with treatment services, including maintaining linkages to AOD and primary health services; functional living supports; and support to maintain community and family connections.
