Service Description

Delivers NDIS services under the person-centred model of care for people who have been diagnosed with a intellectual, physical and psychosocial disorder. Services include Support Coordination and Psychosocial Recovery Coaching; assistance with personal activities and daily activities, Shared living (short term accommodation, supported independent living and independent living options); development of life skills; household tasks; community participation; group and centered based activities; meal preparation; social support; assistance with shopping and attending appointments.
