Chona Care is an independent service provider based in Sydney NSW. Specialising in supporting NDIS participants living with psychosocial disabilities and complex changing medical needs to promote recovery, independence and positive health outcomes. Services include: in home and community supports to include meal planning and preparation, medication supervision supports, prompting with personal care tasks, shopping assistance, attending medical appointments and engaging in chosen social community activities.
