Service Description
A nursing service, as part of Primary and Community Health Services. Services provided include Palliative Care, Advocacy, Assessments, Medication Supervision, Carer Support, Diabetes, and Support and Care After Hospital Stay. Services are for people who are frail, aged, people with chronic illness, people with a disability, and carers.
