Service Description
A community mental health service that provides rehabilitation, recovery and support services to individuals living with severe and persistent mental illness. Works using a Recovery Model to set goals with support of a support worker. There are group programs, outreach services and transition support, and individually tailored support to help people live well and more independently at home and in the community. Services provided to people 16 years and older.
