Community Options Project
Service Description
Provides complex case management to people with disability. Support to further personal development and independence and encourage social interaction. Aged care services for people 65 years and older, 50 years for Aboriginal and Torres Strait Islander people, who need assistance with daily living to remain at home. People aged 50 years and over on low income and at risk of homelessness or who are homeless are also eligible.
