Concord Hospital Palliative Care Service
Service Description
Provides quality palliative care services to people who are living with, or are at end of life with a terminal illness or condition. It assists with quality of life, support and needs physically, emotionally and spiritually for the patient, family and friends. Services are provided by trained palliative care professionals, including doctors, nurses, social workers, occupational therapists, physiotherapists, bereavement counsellors and volunteers. Community nurses care for patients in their own home with pain management and medication. Support for family is available during final stages of life and counselling in the bereavement period.
