Service Description

Provides health services and in home support to older people and their carers across rural, regional and remote areas of Australia. Support encourages independent living and involving the person in their personal care needs. Other services include Home Care Packages, Dementia and Memory Programs, Carers Respite, Meal Delivery, e-health, Transport, Transition Care and Social Programs.
