Service Description
FHC offer a comprehensive range of community-based outreach programs and services designed to help participants maintain their independence and continue living safely in their own homes for as long as possible. We are an approved provider of Support at Home program and Commonwealth Home Support Programme (CHSP). Services are tailored to individual needs and may include Domestic Assistance, Social Support, Respite, Personal Care and Nursing.
