Service Description
The Dementia Awareness & Support Team is a mentoring, information & education service for older people with dementia & people with memory loss who require assistance with screening & assessment. This Community Home Support Program service is available to individuals, couples & family carers. For new referrals please contact Intake on P: 1800 942 433.
