Aged Care Transitional Intervention Program
Service Description
A community aged care program providing short term support, case management and interventions to people who have had an illness or injury that has compromised their independence. The service is available to people 65 years and older with an illness or injury, Aboriginal and Torres Strait Islander people over 45 years of age, and needing a short period of intensive intervention.
