Service Description
Supports people who experience enduring mental illness so that they may access the services and supports that will assist them in their recovery. The Support Facilitator will conduct a detailed assessment of the support required. A plan will be developed and the facilitator will assist with implementing the plan, accessing services, and assist the person to live a meaningful life. An NDIS provider.
