Specialist Palliative Care Clinic
Service Description
A consultancy and liaison service for GPS, Community Nurses, Hospital staff and staff of Aged Care facilities for care and management of clients with a life limiting illness. Services include assessment, pain management, support for end of life care, social support for clients, and access to equipment loan pool. Bereavement support is available, and after hours telephone support.
