Flourish Australia Recovery and Resources Services Program
Service Description
Provides support for people who experience enduring mental illness to access the services and supports that will assist them in their recovery. The Support Facilitator will conduct a detailed assessment of the their needs. A plan will be developed and the facilitator will assist with implementing the plan, accessing services, and assist the person to live a meaningful life. An NDIS provider. Available to people 16 years and older.
