ADAHPS
Service Description
A state-wide public health service for people with HIV related cognitive neurocognitive disorder, multiple comorbidities and complex psychosocial needs. ADAHPS has different eligibility criteria and accepts referrals for people with HIV and complex needs. Services provided include case management, neuropsychological assessment and HIV supported accommodation. Support is available to HIV positive prisoners upon release, and HIV positive individuals who are seen through AOD Integrated Care Programs.

Community support agency via referral form available on website. An assessment will be conducted by telephone and at home visit after referral is received.
