Western NSW Primary Health Network

Service Description

The primary function is to coordinate health services in the area, in order to provide patients with proper care when required. They do not provide services to the public. Works with services to ensure that people at risk of poor health receive proper treatment and have accessible health services. Works with hospitals, Local Health Districts, Health Care Providers, and GPs. Areas include Aboriginal Health, Chronic and Complex Care, Older Person's Health, Maternal and Child Health, Mental Health and Substance Abuse, Early Intervention, Workforce Health, and Integration and Collaboration of services. Public forums are held.
