Service Description
Provides assessment, information, referral and assistance to older people and people with disabilities who wish to remain living in the community or who are considering moving into a Residential Aged Care facility. Provides ACAT, assesses and approves older people for Australian Government Subsidised aged care. Can also help arrange respite care. For people 65 years and older.
