Aged Care Assessment Team Lawson
Service Description
A multidisciplinary team who undertake comprehensive assessment of older clients with complex care needs, while providing information regarding aged care services. Includes Commonwealth Government funded service assessments for Home Care Packages, residential aged care, residential respite and Transitional Care. Younger clients with disabilities may in some circumstances be eligible for ACAT assessment and services.
