Service Description
Provides culturally sensitive Health & Wellbeing services to Aboriginal and Torres Strait Islander people and their families across the Shoalhaven including Wreck Bay (ACT).
Health Services include: GPs, Dental Services, Chronic Care Support, Care Plans, Health Checks, Diabetes Educator, Podiatrist, Hearing Tests, Eye Test, Immunisations and Dietician.
Wellbeing Services include: Case Management, Counselling, Psychology, Child & Youth Mentoring, Child & Youth Therapeutic Services, Men�s, Women�s and Youth Group, SMART Recovery and School Programs to provide support for tobacco use, alcohol and other drugs, and to improve the social emotional wellbeing.
*Transport is available for people who are unable to drive for medical reasons and are attending appointments or specialist appointments.
