Service Description
The Way Back Service - Suicide Prevention and Recovery (funded by PHN). The Way Back Service was developed by Beyond Blue and is designed to support people who have recently experienced suicidal crisis or had an attempted suicide. Provedaalso are a NDIS - Registered Provider of Support Coordination; Ageing - Home Care Package Provider; CHSP funded services Fee for service self-funded packages; Hoarding & Squalor; Dementia Advisor; We also assist people to access the NDIS GP Social Work Program (funded by PHN); Chatter 'Cino - Dementia coffee group; Parent Carers of People with Disabilities Coffee Group, Compassionate Communities Coffee Group
