HIV Outreach Team
Service Description
A mulitdisciplinary team working in local government areas through outreach services. It includes client homes, correctional centres, homeless agencies, or places convenient to the client. They assist with self management towards issues and services to help, such as social services and health, as well as providing advocacy and support. It includes nurses, dietitians, social workers and occupational therapists.
