Service Description

Comprehensive clinical assessment for the frail and aged. Facilitates access to available care services appropriate to their needs. Home assessments are possible through community services. Assists families with appropriate aged care regarding dementia and other mental disorders. Service for 55 years plus, 50 years for Indigenous people.
