Service Description
Assistance given to older people and their carers to determine the appropriate level of care that will best meet their needs if they are unable to remain at home without assistance. Information is provided on suitable care options. An ACAT assessment and approval are required prior to requesting government assistance for subsidised residential or at home aged care.
