The NSW Health â Aged Care Assessment â Nepean team is one of the assessment services covering the Blue Mountains, Nepean, Hawkesbury and Lithgow local government areas; They conduct aged care assessments for all levels of aged care services at home and also assess for residential care. All referrals need to be made through My Aged Care.
