Holbrook Aged Care Assessment Service
Service Description
Provides support and assistance with decisions regarding aged care. An assessment is performed by the Aged Care Connect Team, and recommendations given to family and carers, as well as the person involved. Will assist with appropriate accommodation and care for people who are frail, aged with complex needs. For people 70 years and over, Aboriginal and Torres Strait Islanders from 50 years of age.
