Far West Aged Care Service
Service Description
An aged care assessment is done by ACAT, to assist older people and their families and carers with care options. It offers options for support and care when the person is no longer able to manage at home without assistance. Assistance is provided for suitable care options and will help with access and referral to appropriate residential or community care services, such as HACC.
