St Vincents Community Health Service
Service Description
Provides health services to people 18 years and older within the local area. It includes Social Work, Post Acute Care, Transitional Aged Care, Physiotherapy, Chronic Care and General Counselling. Other services are pre hospital assessment, assessment and referral to other welfare services, HIV/AIDS care, Aboriginal Health, Palliative Care, Medication Assistance and Homeless Outreach.
