Service Description

Provides information and advice to help people in accessing support services in the community. Assists people of all ages including youth, adults, older persons and carers to access disability care, aged care, complex care, mental health, dementia, preventative care and respite options. The ComPacks program is available to support a return home from hospital. Provides community services and referrals to other services in order to assist with regaining independence and avoiding hospital readmission.
