Service Description

Services provided to people of all ages according to needs. It is available at home or through health care centres. Nurses are able care for individuals through Community Nursing, Connecting Care Program for chronic disease such as diabetes, or cardio-vascular, Continence, Hospital in the Home for medication management, such as antibiotics or drop, Palliative Care, and Wound Care.
