Provides nursing care in the person's home or at a Community Health Clinic. Services that nurses provide include wound care assessment and management, Hospital in the Home for medication management, such as drips, Palliative Care, Continence Care, Chronic Conditions, Referrals to Allied Health Services, such as Physiotherapy, Occupational Therapy, Podiatry and Speech Pathology.
