PCCS strengths include Care Coordination, linking and support services for people living with long term health conditions and mental illness, who have complex needs. Call the friendly PCCS team to discuss your needs and find a program to help. Programs include Social Prescribing (Plus Social, Well Together Groups, SKillness), Social Work assistance (GP Social Work Connect) and Mental Health (Head to Health, NewAccess, Psychological Access+), plus a suite of NDIS registered services (NDIS Access and Plan Management, NDIS Support Coordination, NDIS Group/Centre Activities, NDIS Development-Life Skills, NDIS Participate Community, NDIS Assist Access/Maintain Employ, NDIS Assist Personal Activities, NDIS Community Nursing Care 8238, NDIS Therapeutic Supports, NDIS Ex Phys Pers Training, NDIS Assist-Life Stage, Transition).
