Service Description

Supports frail older people, younger people with a disability and their carers to assist them to remain at home. Assists people with complex care needs. Provides a number of programs providing case management services that provide specific needs for frail older people and younger people with disabilities. There are programs for Early Onset Dementia, community awareness, and men's or couples gyms. People over 65 through My Aged Care.
