Service Description
Provides a range of programs and case management services for frail older people, and younger people with disabilities. Disability Awareness Training is available to Disability Employment Services and businesses who employ people with disability. Programs offered include Case management for people from CALD backgrounds and people with dementia, Wellbeing services, Support coordination for older parents and carers, Community Aged Care packages for people from CALD backgrounds.
