Service Description

Support services for frail aged and people with a disability. Through the provision of services, it assists them to remain living independently in their own homes for as long as possible. Services provided include transport bookings, Aged Care Assessment Team (ACAT) referrals, Meals on Wheels, Vital Call, Social Support and functions, and Department of Veteran's Affairs transport claims. Meals on Wheels service is available, and social activities. For ages 65 and above (55 years and above for Aboriginals).
