Service Description
A State and Commonwealth funded service providing a range of services based on individual need and choice for people above 18 years of age. Includes case management, aids and equipment, in-home respite care, assessment, referral, information, meals, home maintenance, and modification. Carer support, community aged care packages and adult day care are available.
