Service Description
Comprehensive assessment of the care needs of frail, older people, using a multi-disciplinary and multi-dimensional approach. A comprehensive assessment of the physical, medical and psychological culture of a client's care needs is performed and a choice of appropriate service needs is provided. Aged care services include Residential Respite, Residential Care, Flexible Care and Home Care Packages.
