Service Description
Occupational and psychosocial assessment for the aged. Therapy, counselling, case management, care co-ordination and advocacy services provided. Information about social groups, respite care and accommodation is available. Other services include Transitional care program, rehabilitation beds, visiting Psychogeriatrician, Social Worker and psychogeriatric visits. The service is for people aged 65 years and older, 55 years for Aboriginals and Torres Strait Islanders.
