Aged Care Access Service

Service Description

A joint initiative of the Commonwealth Government and state and territory governments, to provide integrated health and aged care services for small rural and remote communities. It enables services in regions that would not be able to support stand alone hospitals or aged care homes. It provides continuity of care and mixed services by accounting for local health care needs and resources available.

Contact through My Aged Care either website or phone. After initial contact may be referred to an Aged Care Facility or assessment through ACAT.
