Service Description
Assessment of older persons in order to determine eligibility for Australian government-subsidised aged care services. Older adults, aged 65 and over, or 50 and over if Aboriginal or Torres Strait Islander. Aged care services include entry-level supports under the Commonwealth Home Support Program, Home Care, Residential Care (Respite and/or Permanent). Referrals are made to appropriate services based on the individual needs identified at assessment.
