Service Description
Assessment of older persons in order to determine eligibility for Australian government-subsidised care and support options. Older adults, aged 65 and over, or 50 and over if Aboriginal or Torres Strait Islander. Aged care types include residential care and home care. Referrals are made to appropriate services based on the individual needs identified at assessment.
