Aged Care Assessment Team (ACAT) - Taree
Service Description
Comprehensive dementia, occupational therapy and physiotherapy assessment for the aged. Services include counselling, care coordination, advocacy, respite care and accommodation, podiatry, and a Geriatrician. Aged care nurses provide 12 week home assistance after hospital stay, cleaning, showering and other home assistance. ACAT does assessment for entry to HA funded services. For people 65 years and older, Indigenous people eligible from 50 years.
