Service Description
Assistance for older people 65 years of age, 50 years for Indigenous people, to find suitable support and personal care. An assessment is done to determine level of care required. Support is provided for home care so the person can remain at home with assistance for personal care, transport, home modifications, meals, nursing, or equipment, or residential aged care, nursing homes or respite care.
