Comprehensive Assessment for eligibility to access to Aged Care Services under the Aged Care Act All referrals for assessment from the community required to sent through My Aged Care PH: 1800 200 422 or by Web form at www.myagedcare.gov.au In patient assessments to be sent by hospital treating team directly to ACAT All clients under 65 community and Inpatient to be sent via My Aged Care.
