Service Description
Comprehensive assessment of frail older people, taking account of physical, medical, psychological, cultural and social needs. Provides information regarding community and residential aged care services, including respite care. Approval for eligibility to access Commonwealth funded care programmes, such as Community Aged Care Packages, residential and/or respite care. Service available for frail older people over 65 years of age. Indigenous people are eligible from 50 years of age.
