Access Care Team Sydney Local Health District
Service Description
The point of contact for GPs and staff of residential aged care facilities (RACF), in order to provide support and advice. May arrange support services for resident's care within the RACF by linking services, medical teams and specilaists. They are able to arrange transfers and services for Ambulatory Care, Behaviour Issues, Continence, IV Antibiotics, Palliative Care and Wound Care.
