Service Description

The Aged Care Assessment Team (ACAT) provides assessment, information, advice and assistance to older people who are having difficulty managing at home. They assist the person, family and carers in determining the best options of care when someone is no longer able to manage at home without assistance. Provides information on suitable care options and can help arrange access or referral to appropriate residential care.
