Comprehensive psychiatric, occupational and psychosocial assessment for older people aged 65 years and above. Short term case management, care co-ordination and advocacy. Information about social groups, respite care and accommodation. Outreach to people's homes, Community Care Team, Home therapy, Occupational Therapy and Physiotherapy.
