Service Description

Centralised Intake, Information and Intervention centre for Primary and Community Health Services. It is the single point of access and contact for the General Public, Private and Public hospitals, General Practitioners, other Government and Non-Government Organisations. Services include Community Health Nursing, Acute hospital referrals, Palliative Care, On Call palliative care Support, Continence-limited areas only, Specialist Aged Care Services, Child & Family Health bookings and Aboriginal Chronic Care Program.
