Service Description
This a component of the Rehabilitation Service of Bankstown Community Health Service. A non-clinical service catering for people diagnosed with a mental disorder and people who are socially isolated. This service is an assertive outreach team. Operates through Bankstown Community Health Service, for people 18 years and older who have a diagnosed mental illness and are working with a case manager.
